Diagnostic Evaluation

Treatment begins with knowing which nerves are injured, where along the nerve, and how badly — measured, not assumed.

Electrodiagnostic Testing (EMG and Nerve Conduction Studies)

Performed on site at our St. Louis clinic. Nerve conduction studies measure how quickly and how strongly a signal travels along a nerve; needle electromyography (EMG) records the electrical activity of the muscle that nerve supplies. Together they help identify nerve damage, localize where it is occurring, and distinguish a compressed or entrapped nerve from a metabolic neuropathy — a distinction that changes the treatment plan. Large-fiber testing can be normal in small-fiber neuropathy, so results are always interpreted alongside your history, examination, and the broader root-cause workup.

VNG Balance Testing

Videonystagmography records eye movement to measure how well the balance organs of the inner ear are working. Neuropathy degrades the signal your feet send to your brain, but it is not always the only reason a person is unsteady, and the difference changes what balance work should target. Advanced medical imaging suite used for image-guided interventional neuropathy procedures

Vagal Nerve Stimulation (NS100 Protocol)

Low-level electrical impulses delivered to the vagus nerve to modulate pain signaling and support nerve function. For painful diabetic neuropathy we use the NS100, an FDA-cleared Class II programmable peripheral electrical nerve stimulator. Four small electrode arrays are placed percutaneously in a roughly 30-minute in-office procedure under topical anesthesia and applied to the auricular branches, so the device acts as a vagal stimulator alongside the trigeminocervical complex. They stay in place for a 20-day course and are then removed at a follow-up visit — nothing is permanently implanted. Exosome-Based Nerve Regrowth

Employing exosomes to promote nerve regeneration and repair, facilitating recovery from neuropathic damage.

Orthobiologic Injections (PRP and BMAC)

Platelet-rich plasma (PRP) and bone marrow aspirate concentrate (BMAC), both prepared from your own blood or marrow and delivered under image guidance. Platelets carry growth factors involved in healing and new blood-vessel formation. For peripheral nerve repair specifically, the evidence is at an earlier stage than it is for joints and tendons — the evidence comes from preclinical models and small clinical series rather than large randomized trials, and these therapies are not FDA-approved specifically for peripheral neuropathy. Class 4 Photobiomodulation (High-Power Laser Therapy)

Therapeutic laser light delivered at class 4 power to target cytochrome c oxidase, the enzyme that drives energy production in the mitochondria of injured nerve tissue, and to support circulation through the damaged vasa nervorum. Class 4 systems are higher in power than the class 3B units commonly marketed as “cold laser,” and are selected when greater depth of penetration is needed.

Low-Level Laser Therapy (Cold Laser)

Class 3B low-level laser applied to focal areas of nerve pain. Lower in power than the class 4 system and used where a gentler, more localized dose is the better match for the tissue being treated.

Whole-Body Infrared Therapy

High-intensity infrared delivered from a dedicated treatment table, providing deep, penetrating warmth across the whole body to improve circulation and tissue extensibility and reduce muscle guarding — often used ahead of rehabilitation or a procedure so treatment is better tolerated.
