This Privacy Policy governs how we collect, use, disclose, and protect your personal information when you interact with our website at regenerve.com and related services.

- Contact Information: Name, email address, phone number, and mailing address provided voluntarily through our contact forms or the Nerve Damage Score assessment.
- Assessment Answers: Your responses to the five Nerve Damage Score screening questions, used solely to generate your educational report (no Protected Health Information is collected through our website).

- Communication Preferences: Your opt-in or opt-out status for SMS and email communications.
3. Mobile & SMS Communication Policy
Mobile information will not be shared with third parties or affiliates for marketing or promotional purposes. Exceptions include text messaging originator opt-in data and consent; this information will not be shared with any third parties.
When you provide your phone number to Regenerve, you may receive automated SMS messages related to: appointment reminders, appointment confirmations, appointment status updates, and responses to your inquiries. 4. HIPAA Notice of Privacy Practices
As a covered entity under the Health Insurance Portability and Accountability Act (HIPAA), the Center for Interventional Pain Management / Regenerve maintains strict policies governing the use and disclosure of Protected Health Information (PHI). This website does not collect, transmit, or store Protected Health Information. All patient intake forms, medical history forms, and clinical communications are handled through HIPAA-compliant platforms with appropriate Business Associate Agreements (BAAs) in place.
No Protected Health Information is transmitted via standard email. All forms requiring clinical information redirect to encrypted, HIPAA-compliant portals. 5. Analytics & Tracking
Regenerve does not use third-party advertising trackers, does not sell user data, and does not build advertising profiles. Website analytics are used solely for aggregate website performance measurement and are never used in ways that could be construed as tracking PHI or personal behavior.

We do not sell, rent, or trade your personal information. We do not share your information with third parties for marketing purposes. We may share information with service providers who assist in operating our website and practice, where those providers have signed appropriate data processing agreements and, where applicable, Business Associate Agreements.
